Bring a large pot of water to a boil and cook the macaroni according to package directions until tender.
Drain the cooked macaroni in a colander and set aside to cool completely.
While the macaroni cools, grate the white onion.
In a very large mixing bowl, combine the cooled macaroni, shredded carrots, and grated onion.
Pour the apple cider vinegar over the pasta mixture. Mix thoroughly with your hands to combine.
Cover and refrigerate the pasta mixture for about 30 minutes to marinate.
While the pasta marinates, prepare the dressing. In a separate bowl, add the mayonnaise, milk, and granulated sugar.
Season the dressing with a generous pinch of salt and black pepper, then whisk until smooth and well combined.
After 30 minutes, remove the pasta from the fridge. Pour the dressing over the marinated pasta mixture.
Mix everything together thoroughly with your hands until the pasta is evenly coated. Serve immediately or chill until ready to serve.
