Peel and thinly slice the onions.
Place the sliced onions in a large pan. Add the butter, olive oil, and a hefty pinch of salt.
Cook the onions on low heat for about an hour, stirring occasionally, until they are deeply caramelized, soft, and sweet.
Add the minced garlic, Calabrian chili paste, and crushed red pepper flakes to the caramelized onions. Stir and cook for another minute until fragrant.
Pour in the heavy cream and bring the sauce to a light simmer.
Stir in the grated Parmesan cheese until it melts and the sauce is creamy.
While the sauce finishes, cook the bucatini pasta in salted water until al dente. Reserve about a cup of pasta water before draining.
Add the cooked pasta directly to the pan with the sauce. Pour in about 1/2 cup of the reserved pasta water.
Toss everything together until the pasta is well-coated and the sauce clings to the noodles.
Garnish with a sprinkle of additional crushed red pepper flakes and fresh chopped parsley. Serve immediately.
